What does bone marrow transplant mean?
Bone Marrow Transplant is a type of treatment in which given therapy for cancer or diseases are related to the bone marrow.
throughout the time of this transplant, the donor or the patient’s stem cells of bone marrow are taken out , filtered and given back to the patient. In this procedure, the unhealthy bone marrow will be eliminated and healthy bone marrow cells will be transfused.
In spite of the fact that it is not a standard treatment, Bone marrow transplant is used in the treatment of leukemia, aplastic anemia, lymphoma, immune deficiency disorders, and some tumor cancers.
How many kinds of bone marrow transplant are there?
There are three kinds of bone marrow transplants:
How is bone marrow transplant performed?
How to prepare for bone marrow transplant?
Days admitted :The patient may be admitted in the hospital throughout the treatment of chemotherapy, the duration of which may vary from patient to patient.
Anesthesia : If the patient requires a bone marrow harvest surgery, general anesthesia is executed.
Recovery :– The patient will receive anti-infection medications. – Blood transfusion may also be required. – The patient will need to be fed through an intravenous tube until the mouth sores are cured. – Medication to stop graft versus host disease is also provided. – After that patient is discharged from the hospital, the patient needs to continue home recovery for two to four months. – The patient cannot return to full-time work and normal activities for six months. – For the first several weeks the patient will be too weak to do anything apart from sleeping, sitting and walking inside the house.
Risks :The risks of bone marrow transplant depend on many factors like the type of bone marrow transplant, disease, age and health of the patient etc. Some of the complications are as follows: – Bacterial, viral and fungal infections – Low platelets (thrombocytopenia) and low red blood cells (anemia) – Pain due to mouth sores and gastrointestinal irritation – side effects of chemotherapy and radiation treatment – Overload of fluids resulting in pneumonia, liver damage and high blood pressure – Respiratory distress due to infection, inflammation, bleeding etc.
In the lungs – Organ damage caused by infection, chemotherapy and radiation, graft versus host disease and fluid overload – Graft failure caused by infection , insufficient stem cell count and recurrent disease – Graft versus host disease (GVHD) affecting the lungs, gastrointestinal tract, skin, lungs and liver
After care :– The patient’s white blood cell counts post- transplant are very low so the patient will be very unsafe to infections. – The patient should avoid going to public places during the recovery period. – The patient may need to wear protective masks. – The patient needs to visit the hospital several times a week for monitoring, blood transfusions and medications. – Most patients feel that the quality of life has been improved after bone marrow transplant.

A thorough physical examination is conducted and the entire medical history of the patient is reviewed. A series of various tests are performed in order to review the current condition of the patient and prepare for the right transplant protocol. Incase of an allogenic transplant, donor viability is also confirmed at this stage. The entire process can take 7-10 days. Patient is admitted approximately 7 days before the date of the actual transplant and is called Day-7. He/she is started on treatment to destroy the damaged/diseased bone marrow and given supportive drugs. A hickman’s catheter is inserted in chest area for ease of giving high dose chemotherapy drugs and transfusions. This port helps in preventing damage to the veins in the arms. We are always ready to have a discussion regarding emotional stress with the family in order to have a smooth transplant & recovery process.
The day of transplant is called Day 0 and the stem cells are infused on this day. It takes roughly 15 days for the stem cells to form a healthy bone marrow inside the patient though each patient might respond differently. During this time the patient is kept in a dedicated isolated room with HEPA filters and all nutritional support along with supportive treatment is provided. While we wait for engraftment(new cell formation) some minor complications might occur and are managed religiously by our highly experienced staff. Experience plays a huge role in managing a transplant patients and we value it. An autologous transplant patient stays for roughly 15-21 days in the hospital while the allogenic transplant patient stays for 21-30days. Both patients can be followed up for upto one month on an outpatient’s basis, during which a patient’s blood counts are monitored and dose of drugs is altered accordingly. Post this period you are free to return home and follow-up with us and your doctor via internet or if you wish we can refer you back to your local hematologist.
